Mục tiêu: So với mổ mở, phẫu thuật cắt thân đuôi tụy nội soi có tỷ lệ biến chứng ít hơn, thời gian nằm viện ngắn hơn với những bệnh nhân được lựa chọn thích hợp. Nghiên cứu này nhằm mô tả đặc điểm phẫu thuật cắt thân đuôi tụy nội soi và đánh giá kết quả sớm sau mổ.
Đối tượng và phương pháp: Nghiên cứu thực hiện trên 7 bệnh nhân được mổ cắt thân đuôi tụy nội soi tại khoa Phẫu thuật Tiêu hóa, Bệnh viện Hữu nghị Việt Đức từ 4/2009 đến 4/2013. Mô tả đặc điểm tổn thương tụy, đặc điểm kỹ thuật mổ và kết quả phẫu thuật.
Kết quả: Phẫu thuật được chỉ định cho 1 trường hợp ung thư nang tuyến, 3 trường hợp nguy cơ ung thư (1 u đặc giả nhú, 1 u nang nhầy và 1 u nội tiết), 3 trường hợp nang tụy lành tính. Thời gian mổ ngắn nhất 150 phút, dài nhất 360 phút, 2 trường hợp bảo tồn lách, 1 trường hợp chuyển mổ mở, 2 trường hợp nội soi hỗ trợ. Biến chứng sau mổ có 1 bệnh nhân viêm tụy cấp, không có trường hợp nào rò tụy, không có tử vong trong và sau mổ.
Kết luận: Nếu lựa chọn bệnh nhân thích hợp, phẫu thuật cắt thân đuôi tụy nội soi có thể được thực hiện an toàn với tỷ lệ biến chứng sau mổ thấp.
Preliminary results of laparoscopic distal pancreatectomy

Abstract
Objective: As comparing with open sugery, laparoscopic distal pancreatectomy is associated with lower morbidity and shorter hospital stays with carefully selected patients. This paper aimed at describing operative techniques and evaluating early post-operative results.
Patients and method: Retro-observational review of 8 patients underwent laparoscopic distal pancreatectomy at the Department of Digestive surgery, Viet Duc hospital, from April 2009 to April 2013. Pancreatic lesion characteristics, operative techniques and perioperative outcomes were analysed.
Results: The procedures were indicated for: 1 cystadenocarcinoma, 3 potential malignant tumors (1 solid pseudopapillary tumor, 1 mucinous cystic adenoma and 1 neuroendocrine neoplasm). The operative time ranged from 150 to 360 minutes; 2 patients had a preservation of the spleen. There were 1 conversion, 2 hand-assisted procedures. Post operative complication was detected in 1 patient with acute pancreatitis. There was no pancreatic leak, no perioperative mortality.
Conclusion: With a suitable patient selection, laparoscopic distal pancreatectomy remains a safe procedure with low post operative complication rate.
