In both systems the current flows from the negative terminal of the pacing
box down the pacing lead to the negative electrode and into the heart.
The current is then picked up by the positive electrode and goes back up the
lead to the positive terminal of the pacing box.
Epicardial pacing leads:
Bipolar leads are usually used post op
Bipolar epicardial leads have two separate insulated wires, one
positive and negative
These are sutured to the chamber of the heart that needs to be paced
Both leads are in contact with the heart so either
wire may be used as
the negative or
pacing electrode
The remaining wire is used as the ground or positive electrode
