studies show that berberine induces selective apoptosis through the
AMPKmediated mitochondrial/caspase pathway in HCC. Previous stu-
dies have shown that the expression of Id-1 (inhibitor of di
fferentia-
tion/DNA binding) is associated with proliferation, metastasis, and
angiogenesis of HCC [

]. And the treatment of berberine can ob-
servably decrease Id-1 [

]. These illustrate another role of berberine,
that it could e
fficiently suppress Id-1 expression and inhibit the growth
and development of lung metastases in hepatocellular carcinoma [

].
To sum up, berberine exerts anti-tumor e
ffect mainly through inhibiting
cell proliferation, metastasis, promoting cell cycle arrest and accel-
erating apoptosis in HCC.
Tetrandrine, a bis-benzylisoquinoline alkaloid extracted from the
dried root of Stephaniae tetrandra S Moore, exhibits broad pharmaco-
logical e
ffects and has huge potential to be used as a cancer che-
motherapeutic agent or an adjustment of chemotherapy and radio-
therapy [

]. Based on previous researches, tetrandrine possesses
some typical activities including anti-in
flammation, antioxidant, and
anti-
fibrotic effects, along with the capability of treating high blood
pressure, arrhythmia and arthritis mentioned in some researches
[

,

]. What is more, tetrandrine has shown anti-proliferation e
ffect
via regulating the cell cycle, Wnt/
β-catenin-mediated pathway, autop-
hagy and angiogenesis in various organs including renal, lung, colon
and liver [

]. Previous studies have suggested that tetrandrine sig-
ni
ficantly decreases the level of the vimentin (mesenchymal marker)
but increases the expression of the occludin and E-cadherin (E-cad) in
HCCLM9 and Huh7 cells [

]. The results have shown that tetrandrine
inhibits the invasion and migration of HCC cells, which is related to the
inhibition of EMT [

]. Several antitumor drugs have been a
ffirmed to
inhibit tumor migration by inducing autophagy, which is a double-
edged sword in tumorigenesis and metastasis in cancer cells [

]. Thus,
some researchers have indicated that tetrandrine can indeed inhibit the
metastasis of hepatoma cells by autophagy [
65
]. In addition to de-
creasing p-GSK3
β and Wnt3a protein levels, and inhibiting β-catenin
transcriptional activity inhibited-catenin translocation to the nucleus,
tetrandrine could also increase the p-
β-catenin protein and the accu-
mulation of
β-catenin in the cytoplasm [

]. These results have well
explained that autophagy-dependent Wnt/
β-catenin signaling is in-
volved in tetrandrine-induced inhibition of HCC cells migration. Me-
tastatic tumor antigen 1 (MTA1) is associated with tumor invasion and
migration and generally is highly expressed in cancer, such as in liver
cancer [

]. Tetrandrine treatment obviously decreases the expression
of MTA1, which indicates that tetrandrine could inhibit HCC migration,
in part through MTA1 [

]. Moreover, tetrandrine increases the ex-
pression of LC3-II, recognized as a membrane-bound form of LC-3 and
an established marker of cell autophagy in HCC cells, which demon-
strates that tetrandrine could induce autophagy. And further study has
proved that tetrandrine-induced autophagy is mediated by the pro-
duction of intracellular ROS following mitochondrial depolarization
[

]. The levels of phosphorylated ERK1/2 signi
ficantly increases after
tetrandrine treatment, but no obvious changes are found in the levels of
phospho-P38 MAPK, AKT or JNK, indicating that tetrandrine-induced
autophagy in liver cancer cells is dependent on ERK activation [

].
Moreover, it is observed that tetrandrine treatment increases the pro-
tein expression level of ATG7 in HCC cells [

]. ATG7 is reported to be
a core regulator of autophagy controlling the biogenesis, size and
quantity of autophagosomes [

]. These results reveal that ATG7
and ROS/extracellular signal-regulated kinase contribute to tetran-
drine-induced autophagy in HCC cells [

]. Besides, tetrandrine can
induce cell cycle arrest by distributing the cell populations in G1, S and
G2 phase. Tetrandrine treatment could increase the expression of p21
and decrease the expression of PCNA [

]. PCNA is synthesized in the
G1 and S phases and p21 could control the cell cycle progression from
G1 to S phase, which play an important role in the regulation of the cell
cycle. Tetrandrine plays an anti-cancer role not only by triggering au-
tophagy, inhibiting cell migration and invasion, inducing cycle arrest,
but also by inducing apoptosis. It is observed that tetrandrine
e
ffectively induces cleavage of Caspase-3, Caspase-8, Caspase-9 and
PARP, which play vital roles in cell apoptosis and DNA repair. These
suggest that tetrandrine-induced HCC cell apoptosis is in a caspase-
dependent manner [

]. Tetrandrine treatment signi
ficantly down-
regulates Mcl-1, c-FLIP (L), Bid and Bcl-2 in both BEL7402 and Huh7
cells [

]. Further, the ROS level increases in HCC cells when treated
with tetrandrine and ROS generation is the upstream event of the AKT
in tetrandrine-induced HCC cell apoptosis [

]. From what has been
discussed above, tetrandrine induces apoptosis by activating ROS and
repressing AKT activity in human HCC.
Colchicine, as an isoquinoline alkaloid agent originally extracted
from the lily family colchicum, has been widely used clinically to treat
acute gout and other immunologic diseases such as scleroderma, psor-
iasis and behcet disease [

]. A well-known anticancer mechanism of
colchicine attributes to disrupting the assembly dynamics of micro-
tubules between colchicine-tubulin interactions [
73
–75
]. In addition,
colchicine also plays an e
fficient role against cancer via interaction with
microtubules, inhibiting the movement of intracellular granules, as well
as some leukocyte functions when it comes to adherence, motility,
chemotaxis, expression of molecules of cellular adherence, and lyso-
some degranulation [

]. Several researchers have indicated that
cancer cell metastasis is in
fluenced by cell deformability, which is as-
sociated with sti
ffness [

]. Some studies have shown that colchi-
cine could increase the sti
ffness of SMMC-7721 cells, decrease the
stability of cytoskeleton, inhibit the cells division and proliferation,
block the polymerization of microtubules, hinder the cell cycle pro-
cession, and regulate some genes expressions. These contribute to its
anti-cancer e
ffects on liver cancer cells, including downregulations of
SEPP1, C4BPA and HSD11B2, and up-regulations of TGFB2, AKAP12
and MX1 [

]. Among them, AKAP12 acts as a tumor suppressor,
which inhibits cytoskeletal pathways and serum creatinine mediated
oncogenic signaling, and regulates cell cycle progression [

]. As we all
know, sti
ffness of cancer cells is adverse to their metastasis and moti-
lity. Colchicine also causes downregulation of APOH but up-regulation
of growth di
fferention factor 15 (GDF15) and IL32 (involved in the
progression of HCC) in HCC cells [

]. Suppression of IL32 gene in-
hibits cell growth and inducts intrinsic apoptosis via inactivation of NF-
κB and Bcl-2 signal, meanwhile inhibition of GDF15 decreases DNA
synthesis, suppresses cell proliferation, and weakens invasiveness of the
HCC cells [

These studies show that the anti-liver cancer e
ffect of isoquinoline
alkaloid not only in
fluences proliferation, apoptosis, autophagy, but
also cell cycle and metastasis. Meanwhile, the pathways involved in
these phenomena include AKT pathway, AKT/FoxO3a/Skp2 axis, PI3K-
AKT-mTOR pathway, Wnt/
β-catenin-mediated pathway and AA meta-
bolic pathway (
Fig. 4
).
2.3. Indole alkaloids
Evodiamine as an active ingredient of many traditional medicines, is
extracted from dried near mature fruit of Evodia rutaecarpa [

which possesses the properties of anti-in
flammatory, analgesia, anti-
emesis, vascular dilatation, and antitumor e
ffect. Clinically it is used for
the treatment of diarrhea, oral ulcer, osteoporosis, beriberi, headache,
postpartum hemorrhage, gastrointestinal disorders as well as cancer
[

]. According to related literature reports, evodiamine could
induce apoptosis in a dose-dependent manner under hypoxia, which
can be proved by the increase of cleaved PARP under hypoxia in HCC
cells [

]. Hypoxia-inducible factor 1
α (HIF-1α) recognized as a crucial
regulator of hypoxia-dependent transcriptional responses and glyco-
lysis, is overexpressed in various cancer cells such as in HCC cells, along
with indication of poor prognosis [

]. The level of HIF-1
α protein
decreases in HCC cells after treated with evodiamine [

]. In addition,
evodiamine/vorinostat combination suppresses the expression of Mcl-1
and Bcl-2, which shows that evodiamine observably enhances the
vorinostat-induced apoptosis in HCC cells under hypoxia [
